Visit Details Change Log

The Visit Details Change Log displays the list of cases that have Diagnosis or Procedure Code changes in Rapid that are not in ENCOR Abstracted, causing a discrepancy between ENCOR Abstracted and Rapid. The report is available in Meditech Systems only. It is not available in the Cloud Application. The report is available from the Data Quality Menu under Reporting.

Select quarter and hospital, click Go.

Cases with differences will have the differences highlighted in yellow.

These cases can now be edited in the worksheet to add or remove diagnosis or procedure codes. CPT Code and E/M code cannot be edited in the worksheet, so cases with those differences or Inpatient to Outpatient or vice versa, would need to be deleted and allowed to return with the correct codes.

Note: If a case is excluded by sampling (Visit Status will display this), it will not be sent anywhere, so unless it has completely coded out of a population, it can remain as is.
