- Cases by Category Assignment
The Cases by Category Assignment Report aggregates the organization’s cases by Measure Status and Measure Set. This report can be used to identify Potential Outliers and to identify any cases that are Measure Category Assignment ‘Y’ (UTD Value) which is considered missing data. - Composite Performance
The Composite Performance report lists the overall performance by Measure Set ID. The Report displays the Inpatient Rate Measures Compliance. It considers reverse Measures and does not double count for the overall and the strata. It displays Good Values (number of Measures that passed) over All Values (number of Measures that were qualified for) and the Composite Performance Rate for each Measure Set. The All Values are all Measures that have been qualified for in that Measure Set.
Note: Composite Performance Rate for a Measure Set is calculated differently depending on whether the Measure Set contains any Reverse Measures. For Reverse Measures a reduced or lower rate is desirable. - Continuous Measures Results
This report generates a listing of time-based measures. This report is used to review Median Values and identify Extreme Values. - Measure Decision Point
The status of a case - e.g. whether it is assigned to the Numerator, Denominator, is Excluded or Rejected, depends on the values entered during abstraction, and how these values relate to the algorithm provided by CMS. In situations where the case fails the intent of the Measure, abstractors often like to know why the case was assigned the particular status. The Measure Decision Point Report gives you information about the last element on the Abstraction Worksheet that was entered which gave the case its measure status (Measure Category Assignment). - Perfect Care
The Perfect Care Report allows you to identify patients that received Perfect Care. It displays Numerator and Denominator and the Perfect Care Rate for each Measure Set. The Numerator is the number of patients that received perfect care, which means that every measure they qualified for, passed. The Denominator is the number of patients that qualified for at least one measure in the Measure Set. - Potential Duplicate Accounts
This report will identify cases that could be potential duplicates in ENCOR Hospital Abstracted Measures. These cases may reject at submission time and may need to be deleted. If cases are deleted, the facility may need to abstract additional cases or re-abstract any cases that may return to abstraction. - Potential Outlier Report
This report will identify only Potential Outliers. This report is similar to Cases by Category Assignment report, with a default filtering for Potential Outlier=Yes. - Quality Measures
The Quality Measures Report provides a combined view of the Rate Measures and Continuous Measures Reports displayed by month. Unlike the Individual Rate Measures Report and Continuous Measures Report which are aggregated by Measure, this report presents aggregated data by measure and month. The drill-down to patient level data is not available on this report. - Rate Measure Results
The Rate Measure Results Report displays all the rate measures – those that have a numerator and denominator. By default, the report displays the data Grouped by Measure ID. You can drill down to patient level data. - Submission Edit Checks
This report will display Inpatient and Outpatient Critical and Informational Edits that will occur at HQR during submission of data. It is based on the QualityNet.org published edits documents. - TR Report
The TR Report displays the Rate for TR cases, including the additional discharges added via the TR Multiple Records Per Visit grid. - Visit Details Change Log (Non-Cloud Only)
The Visit Details Change Log displays the list of cases that have Diagnosis or Procedure Code changes in Rapid that are not in ENCOR Abstracted, causing a discrepancy between ENCOR Abstracted and Rapid. It is not available in the Cloud Application.
Population
- Base Data Import Changes (Cloud Application Only)
The Base Data Import Changes will display the most recent data element changes from the IPPS and OPPS file import for EA Cloud clients. ONLY the most recent import is evaluated against the pre-import data. Clinical files are not evaluated on this report. - Case History
The Case History Report gives you an overview of each case – when the case first came into ENCOR (Date Created), the Service and Admit Date and so on. The report will show the entire list of cases in all populations for the quarter. - Cases No Longer In Population (Cloud Application Only)
The Cases No Longer In Population report will identify any cases which no longer qualify for the measure set you are running. This includes cases that are Completed, In Progress, Not Started, Excluded by Sampling and Locked cases. If they do not belong in the population they are currently in, they will appear on the Report. This report does not show a comparison of the changes between original import and new import. - Cases No Longer in Population Summary (Cloud Application Only)
Cases No Longer in Population Summary Report will reflect the latest counts from Cases No Longer In Population report for all hospitals and all Measure Sets in the organization. It runs once per week. - Event Rate Results
The Event Rate Results report shows quarterly aggregate data for Numerator, Denominator, Leave Days, and Inpatient Days for HBIPS Events measures. Leave Days and Inpatient Days are manually entered by the facility. The data entry grids are accessed by selecting the icons on the Events grid on the Abstraction page. - Excluded Visit Analysis
The Excluded Visits Analysis report shows the aggregate counts including the Numerator, Denominator, and Exclusions for a particular measure. This report is primarily used to obtain the PC-01 data to enter into the QualityNet Portal each quarter. - HF Population
Get With The Guidelines (GWTG) Interface for Heart Failure (HF) Patient List. Transfer patient level HF data to GWTG using the HF Patient List Report. - HF Sampling
The HF Sampling process will perform quarterly sampling on the cases in the HF Population. - IPF Data
This report displays IPF Non-Measure Data, Rate Measure Results and Event Rate Results for HBIPS-Global, SMD, TR-1 and HBIPS-EVT for the entire IPFQR reporting period or for one quarter at a time. - Monthly Abstraction Status
The report displays three months of abstraction status in chart format. - Outpatient Web-Based Measures Report
The Outpatient Web-Based Measures Report is used for reporting the Data Collection encounter period for OP-29 & OP-31. These results will be obtained using this report and entered via the Structural/Web-based measures section of the Hospital Quality Reporting (HQR) website by your QualityNet Security Administrator. - Patient Population
The Patient Population Report provides a complete listing of all cases in the Initial Patient Population. It is very similar to the Case History Report. It can be used to see when a case was last modified by looking at Date Last Modified and when it was sampled by looking at Date Sampled. - PC-01 Quarterly Submission Report
Displays PC-01 data for entry into the HQR PC-01 Data Form. - Quarterly Population and Sampling
This report provides Population and Sampling counts per Quarter listed by month. Use this report to see if your facility meets the sampling requirements. - Rate Measure Outlier Report
This report displays Rate Measure outliers in order to better understand the reason the cases are failures. - Sepsis Bundle Report
The Sepsis Bundle Report displays the breakdown of SEP-1 Composite into four component bundles. Sep-1 Composite, Severe Sepsis 3-hour Bundle, Severe Sepsis 6-hour Bundle, and Septic Shock 3-hour bundle.
Provider
- Provider Attribution
The Provider Attribution Report displays provider specific data by measure. It is also useful to compare a Provider’s performance in Rate Measures and Continuous Measures against the Provider's peers. The data is presented by provider and measure. - Provider Compliance
The report displays the compliance rate by physician. This means the rates have been reversed for the inverse measures, such as PC-01, to provide a compliance rate.

- Abstracted Data Elements Export
The report will generate an Excel spreadsheet of cases that have been completed for a measure set. - Annotations
The Annotations Report displays all Abstraction Completed and Abstraction In Progress cases where annotations exist. The Annotations are pulled from the worksheets where they have been entered by the abstractors. - Deleted Visits
Visits that are deleted appear on the Deleted Visits Report. Deleted visits are not reported to CMS/TJC. - GWTG Report
The GWTG Report displays the cases sent from ENCOR Hospital Abstracted Measures to Get With the Guidelines (GWTG). - Print Worksheet
The Print Worksheet option gives users a handy way to print out blank worksheets for different measure sets and different versions. This gives you an easy way to see what questions are included for a measure set or maybe even compare worksheets to see what changes have been made from a previous version to a current version. - User Access Report (Cloud Application Only)
The User Access Report displays all user listed in ENCOR Abstracted.

Additional Reports are available in the Administration Section. Only Administrators have access.
- Abstraction Count Report
The report displays the number of abstractions saved during a specified time. - Abstractor Productivity Report
The report provides statistics concerning abstraction time by abstractor. This includes Total minutes and Average Minutes for Completed and In Progress worksheets. - Audit Trail
The report displays the activities of all ENCOR Abstracted users for a selected timeframe. - Case Lock Look Up Report (Cloud Application Only)
This report allows a facility admin to look up the lock history of a case. - Cases Not Qualified for Any IPP (Cloud Application Only)
This report will display a list of cases uploaded through the file import process that did not qualify for any enabled measure set. - Clinical File Rejections (Cloud Application Only)
This Report will give you a listing of all cases that were rejected on Import of Optional Clinical Files and the reason. An email notification will be sent to the designated technical point of contact after each file upload. The purpose of the notification is to confirm the processing of your files and to alert you to Duplicates and/or Rejections. - Generate Patient Population (Cloud Application Only)
The Generate Patient Population Report will display a list of the cases in abstraction. This can be helpful when selecting cases to generate optional clinical files. - Import ENCOR Summary (Cloud Application Only)
The Import ENCOR Summary Report is located on the Facility Admin menu. It displays the number of records received, rejected and uploaded. - Invalid Import Results (Cloud Application Only)
This Report will give you a listing of all cases that were rejected on Import of Base Files and the reason. An e-mail notification will be sent to the designated technical point of contact after each file upload. The purpose of the notification is to confirm the processing of your files and to alert you to Duplicates and/or Rejections. - Joint Commission DDSP Data Entry
This report will provide data to be entered into The Joint Commission’s (TJC) Direct Data Submission Platform (DDSP). - User Access Audit Trail
The report displays the access changes for all ENCOR Abstracted users for a selected timeframe. - Workflow Audit
The report displays the sampling workflow for each Measure Set. The report shows where each measure set is in the sampling process (Selected, Marked for Abstraction, Sampled, etc.) and what actions were taken prior to the current step.
