Inpatient Readmission Rate View

This view calculates the Re-admission Rate for a group of patients based on user selected clinical criteria and readmit intervals. All readmissions to an inpatient facility within the health system within 30 days of discharge from an inpatient facility are included. Only the first readmission visit after the initial (index) discharge is counted towards the Re-admission Rate. This view allows users to filter by Facility, Location, Attending Provider, Primary Surgeon for the first Surgical Procedure, Principal Diagnosis, Principal Procedure and 3,7,14 or 30 days from the discharge to the readmission within the specified time period.
