      The Hospital Information page, located on the Facility Admin menu, will display the Hospital Name, CCN, HCOID, if applicable, as well as other facility information.

      • This Hospital has Clinical Trials - ‘Y’ will default Clinical Trial on every worksheet to ‘Yes’, ‘N’ will default Clinical Trial on every worksheet to ‘No’. N/A will leave this question blank on every worksheet.
      • This Hospital uses Skip Logic checked will default every worksheet to using skip logic. The option to turn off skip logic is available on every worksheet.
      • The Stroke Certifications checked allow CSTK-02 or CSTK-10 event grids to populate.
