HF Population
Get With The Guidelines (GWTG) Interface for Heart Failure (HF) Patient List
Transfer patient level HF data to GWTG using the HF Patient List Report.
- Select the quarter using the drop-down and select any or all hospitals.
When the report is displaying a sample, a Note indicating this is displayed.
- Cases that have been selected for the sample will have 'Yes' in the Is Sampled column, while cases not selected for the sample will display 'No' in that column.
- Once results are generated, cases can be transferred to GWTG. Only cases that are in the sample for sampled populations will display the GWTG transfer checkbox.
- The GWTG Column is present on the report. Abstractors can select cases individually or check the box next to GWTG to select all cases.
- Once the selections have been completed, you can Click on the Blue Submit to GWTG button at the bottom of the report.
- After GTWG transfer is completed, a green flag will appear next to the GWTG selection box that will indicate which cases were transferred to GWTG.
