Access the eCQM Add Custom Medications page from the Hospital menu dropdown.
eCQM Add Custom Medicaitons allows you to add a branded drug RxNorm code to a value set in which the exact same clinical drug (with a different RxNorm) is already included in. Per CMS, the value sets almost always contain only the Clinical Drug name and RxNorm. The same Branded Drug may be used in your hospital but can have a different RxNorm. Without adding the code for the branded drug to the value set, patients may not qualify correctly for the eCQMs.
Note: This is the only situation in which this function can be used. If you use this tool to add branded drug RxNorms to the applicable value set, expect to have to defend mappings on audit and have sound technical and clinical knowledge to support any mapping.
There are two options to view and add codes in the custom meds tool. The first tab allows you to search by a specific code, the second tab displays all meds included in a value set. You can use either tab to complete the following steps:
- Find the value set you need to add an RxNorm code to by searching by OID, Code, etc.
- Select the book icon and in the window that displays, enter the following:
The OID you are adding a code to (OID field)
- The RxNorm Code from your formulary that you are adding to the value set (Alias Field)
- The RxNorm Code that you are mapping to (the code currently in the value set) (RxNorm Field)
- The Description of the med from your formulary. The description needs to be an exact match to the description of the med in the value set, if not, you should not map this medication.
3. Click the disk icon to save.
Note: Upon saving, the additional med, the Custom Flag field displays 'Yes' and the last updated and edit options are presented.
Note: Value sets and specifications are updated annually so you need to plan on reviewing and updating custom med mappings regularly. Custom meds can be sorted by the Custom Flag for a quick viewing and can be exported using the gear icon functions.
