Cases No Longer in Population

This report will look at any case in the Initial Patient Population (IPP) table and run IPP algorithm for that measure set to make sure the cases in that measure set still belong there.

This includes cases that are Completed (Abstraction Completed), In Progress (Abstraction In Progress), Not Started (Abstraction Not Started) and Excluded by Sampling (Case is in Initial Patient Population, but not selected for the random sample).

It is on the Population menu under Reporting. The report is available in the ENCOR Cloud Application. It is not available in MEDITECH Systems.

  • Select the quarter and measure set from the drop-down menus, click Submit.


  • Most cases that appear on this report will need to be deleted. You can delete them from this page. Instructions for deleting from this page follow tips below

You may need to bring in various Data Elements from the Field list to verify data elements for some measures (i.e. CPT codes, Discharge Disposition).

      • We have included some tips for data validation while using the Cases No Longer in Population Report.

Tips:

OP-ED: E/M Codes are required for OP-ED and OP-STK. If you see many Cases No Longer qualifying for these Measure Sets, double check the Data Source file included the required E/M Codes.

OP-29: OP-29 requires a CPT Code. If a CPT Category Modifier (52, 53, 73, 74) was added, the case will no longer qualify per the specifications. If you see many cases no longer qualifying for this Measure Set, double check the Data Source file included the required CPT Code and did not have a modifier added.

Cases excluded by sampling: Cases on the Cases No Longer in Population report can be in the sample or excluded by sampling (in the Initial Patient Population, but not selected for the random sample).  They were not originally included in the sample when they qualified for the population, which is why the report says excluded by sampling. These cases are still being counted in your population and need to be deleted if they do not qualify for the population.   You will find Excluded by Sampling cases on the Case History Report.

Large Numbers of Cases on Report: Cases that switch from Inpatient to Outpatient or vice versa OR have their Procedure Codes removed OR have secondary codes removed in the base file import may result in a large number of cases appearing on this report. Before deleting large numbers of cases, please check with your Medisolv ENCOR for Hospital Abstracted Measures Clinical Quality Advisor.

STK/CSTK: STK or CSTK cases may have been recoded and now qualify for a different subpopulation. For example, Ischemic to Hemorrhagic or vice versa. In this case, the current worksheet must be deleted in order for the correct worksheet to populate the tool.  Deleting from the Cases No Longer in Population Report will remove the case from the measure only. The underlying data remains in the staging table and creates the new worksheet in the correct subpopulation when the IPP runs overnight. Subpopulation is available in the field list to confirm the current subpopulation.

To delete cases from this page after you have run the report and cases no longer in population have been identified:

1. Check the box(es) next to the case(s) you wish to delete.

2. Click Delete Cases.

3. The below window will pop-up. Enter a Reason for Delete (such as case no longer qualifies for measure or case recoded) and click Delete from this Measure Set Only.  If a case qualifies for more than one measure set, you will be notified as per the below screenshot.  You can then run the report for the other measure sets to see if the case is present on those reports.  If so, you can delete it from the other measure sets as well.

4. The case is deleted and the pop-up disappears.  

Deleted cases will appear on the Deleted Visits Report.
