Acute Inpatient Readmissions View

This View allows identification of Re-admitted patients based on user defined clinical criteria and readmit intervals. This view allows the user to specify all readmissions either with a specific DRG or MDC who were readmitted to a user defined Acute Care Facility after discharge from Acute Care Facility (the Index or initial Admission) within the user defined number of hours, days, weeks or months over a user defined time period. Several other “Filters” are available for the user to narrow down the search for readmitted patients. Re-admissions can be grouped according to several “Dimensions” included in the Field List e.g. Initial Attending Provider.
