• Minimum of any continuous 90-day reporting period
• Minimum of 60 points required
• Security Risk Assessment and assessment of all nine SAFER Guides will be required but no points assigned
• eCQM measure submission required but no points assigned.
• Failure to report on all of the measures will result in failure to meet the program requirements.

Public Health and Clinical Data Exchange

Report all 4 measures or applicable exclusions for 10 points 

• Syndromic Surveillance 
• Immunization Registry
• Electronic Case Reporting
• Electronic Reportable Laboratory Results

Bonus:  Report 1 of the following measures for 5 points

• Public Health Registry Reporting

• Clinical Data Registry Reporting

Electronic Prescribing

• ePrescribing 10 points
• Bonus:  Query Prescription Drug Monitoring Program 10 bonus points

Provider to Patient Exchange

• Provide Patients Electronic Access to Health Information 40 points

Health Information Exchange

Option 1

• Support Electronic Referral Loops by Sending Health Information 20 points
• Support Electronic Referral Loops by Receiving and Reconciling Health Information 20 points

Option 2

• Health Information Exchange Bi-Directional Exchange 40 points
